Belk Benefits Program Overview

Belk strives to offer a quality benefits program that meets the varying needs of its Associates. Associates have different benefits needs and require the flexibility to choose different types and levels of benefits coverage. The BelkChoice Benefits Program provides that flexibility to Full-Time associates. If you are a parent, your benefits needs are probably different from an Associate who is single or one who is married without children; you need more flexibility in choosing your benefits and more control over how much you spend for benefits.

With BelkChoice, you can select the level of participation that meets your unique needs. Another advantage of BelkChoice is that in many cases your benefits costs can be paid with pre-tax dollars. Using pre-tax dollars to pay for benefits helps make your coverage more affordable. This is because the tax savings you receive helps to offset part of the cost of those benefits. Most benefit plans have a three month waiting period.

The BelkChoice program includes a variety of benefit plans, ranging from healthcare, income protection, survivor protection, rest and relaxation and retirement security. It offers "Choice" benefits, which are available to you on an optional basis and "Core" benefits, which Belk automatically provides to you at no cost.

CORE Benefits (1)

Belk automatically provides substantial benefits to eligible Associates at no cost. These are referred to as "Core" benefits. These are benefits which meet the needs of most Belk associates.

Basic Life Insurance (a) top
Belk provides company paid Basic Life insurance coverage equal to one times your annual pay up to a maximum.
Basic Short-Term Disability Income Insurance (b) top
Belk provides company paid Basic Short-Term Disability (STD) Income Insurance coverage to all eligible full-time associates. The short-term disability income insurance plan provides you with a short-term income source in case you become disabled due to accidental bodily injury, sickness, mental illness, substance abuse or pregnancy.
Employee Assistance Program (c) top
Associates and their qualified dependents are eligible to receive up to three counseling visits per year at no cost. The service is totally confidential - no personal information is shared with the company or anyone else - and all you need to do to access services is to call the toll free telephone number.
Merchandise Discounts (d) top
Another valuable benefit provided to you by Belk is a merchandise discount on most items purchased from Belk stores. Your discount will provide you with exceptional values on quality merchandise.
Vacation/Holidays (e) top
Belk provides time away from work, so you can relax and spend time with your family and pursue hobbies and personal interests. Belk provides paid holidays and vacation, depending on your length of service.
Paid Sick Leave (f) top
You can earn paid sick leave each year. Your sick leave benefit is designed to pay you even though you are absent from work because of personal illness or injury, or because you need to provide care for members of your immediate family who live with you.
Jury Duty Pay And Funeral Leave Pay (g) top
You may receive the difference between your base pay and the compensation you receive while serving on jury duty. Belk also provides eligible associates with time off with pay to attend the funeral of an immediate family member.
Leave Of Absence (h) top
You may request a leave of absence if you need to be away from work for an extended period of time for personal and family needs. If you are granted a leave, service with the company will be continued and many benefit programs in which you are participating (such as medical, dental, discounts, and life insurance) can also be continued.
Business Travel Accident Insurance (i) top
This plan provides accidental death or dismemberment coverage while you are traveling on business for Belk. The maximum benefit depends on your annual pay.

 

CHOICE Benefits (2)

The BelkChoice benefits program allows you, as a qualified Full-Time Associate (scheduled to work 30 or more hours on a regular basis per work week), to choose the coverage that best meets your individual needs in a number of different plans. You choose which benefits best meet your needs, how much coverage you want and who you will choose to cover. The key is you get to make the choice.

Medical Plan (j) top
You may choose between two plan options. You may choose medical coverage for yourself and your eligible dependents. You pay your share of the premium cost through pre-tax payroll deductions.
Dental Insurance (k) top
You may choose between two plan options. Both options emphasize preventive dental care by providing you and your eligible dependents with coverage for services as checkups, cleanings and x-rays. Both options cover the same types of dental services. You may choose the option you want based on the level of coverage you need for such services as fillings, extractions, crowns, etc. and the cost you want to pay. Belk shares in the cost of your personal coverage. You pay your share of the premium cost through pre-tax payroll deductions.
Vision Plan (l) top
Belk's Vision Plan is designed to protect your visual wellness. You may choose coverage for yourself as well as your eligible dependents. The Belk Vision Plan provides coverage for an annual eye examination to assess your visual functions and prescribe corrective eyewear, if needed. Your benefits under the plan are maximized when you use a participating network provider, but you may use any optometrist, optician, ophthalmologist, or other licensed and qualifed vision care provider. You pay the cost of the coverage for you and your dependents through pre-tax payroll deductions.
Health Care Flexible Spending Account (m) top
Through this account, you can reimburse yourself on a tax-free basis for out-of-pocket medical, prescription, dental, and vision expenses, as well as certain eligible over the counter medicines. Once enrolled, you will automatically be sent a Health Care Card (i.e. a Debit Card with No PIN required) within a few weeks of the start of your coverage period. The Health Care Card allows you to access your health care spending account funds at the point of sale, such as at your doctor or pharmacy. A participant can request that payment be made from their HCSA and sent directly to the provider indicated by the participant at the time of request. Or a participant can submit a claim by fax or by mail to get paid or reimbursed for an out-of-pocket Health Care Expense.
Dependent Care Spending Account (n) top
Through this account, you can reimburse yourself on a tax-free basis for child care or disabled adult care expenses while you work. A participant can request that payment be made from their DCSA and sent directly to the provider indicated by the participant at the time of request. Or a participant can submit a claim by fax or by mail to get paid or reimbursed for an out-of-pocket Dependent Care Expense.
Optional Life Insurance (o) top
You may purchase Optional Life Insurance coverage in increments of one to six times your annual pay. You have the option of paying for your coverage through either pre-tax or after-tax payroll deductions. Premiums are based on five-year age brackets.
Dependent Life Insurance (p) top
Associates eligible for Basic Life Insurance coverage may also apply for life insurance coverage for their eligible dependents.
Personal Accident Insurance (q) top
Belk offers insurance coverage to protect you and your family members from accidental death or dismemberment. You may choose coverage for yourself, regardless of your level of pay. Family coverage amounts are based on a percentage of the amount of coverage you choose for yourself. This plan provides you with inexpensive protection which you pay for with pre-tax payroll deductions.
Optional Short-Term Disability (STD) Income Insurance (r) top
The Short-Term Disability Income Insurance plan provides you with a short-term income source in case you become disabled due to accidental bodily injury, sickness, mental illness, substance abuse or pregnancy. You may purchase additional income protection by enrolling in the Optional STD plan. Optional STD coverage is combined with Basic STD coverage to provide a greater benefit. You pay for this coverage with after-tax payroll deductions.
Long-Term Disability Income Insurance (s) top
You may also choose income protection against long-term illness or injury. The long-term disability plan can provide monthly income during your disability. You pay for this coverage with after-tax payroll deductions.
Auto/Home Insurance and Legal Services Plan (t) top
Through an arrangement Belk has with MetLife, you may purchase Home and Auto Insurance or coverage under a group Legal Services plan at special group rates. Your premiums may be paid through the convenience of bi-weekly payroll deductions.

401(k) Savings Plan (u) top
Belk offers a 401(k) savings plan to help provide for financial security at retirement. You can save up to 60% of your eligible pay on either a pre-tax or an after-tax basis or a combination of the two. Belk will match 100 percent on the first four percent of associates' pre-tax contributions and 50 percent on the next two percent of pre-tax contributions for a maximum match of five percent.

The Part-Time Associates Benefits Program allows you, as a qualified Part-Time Associate, to choose which benefits best meet your needs, how much coverage you want and who you will choose to cover. The key is you get to make the choice.

Limited Medical Plan (9) top
You may choose between two plan options. You may choose limited medical coverage for yourself and your eligible dependents. Your Limited Medical Plan is an indemnity plan, however, it is not a major medical plan, nor is meant to replace a major medical plan. This plan does not require a member to utilize a PPO Network, however, if you use a network provider, you can reduce your out-of-pocket expense. You pay the cost of coverage for you and your dependents through pre-tax payroll deductions.
Dental Insurance (10) top
You may choose between two plan options. Both dental options are designed to encourage regular checkups by covering preventive dental work without a deductible. Both plans cover Basic Procedures, however, only the High Plan covers Major Procedures. Neither plan covers orthodontia. This plan does not require a member to utilize a PPO Network, however, if you use a network provider, you can reduce your out-of-pocket expense. You pay the cost of the coverage for you and your dependents through pre-tax payroll deductions.
Vision Plan (11) top
Your Vision Care Plan provides coverage for an annual eye examination to assess your visual functions and prescribe corrective eyewear, if needed. Your benefits under the plan are maximized when you use an in-network doctor but you may use any optometrist, optician, ophthalmologist or other licensed and qualified vision care provider. You may choose coverage for yourself as well as your eligible dependents. You pay the cost of the coverage for you and your dependents through pre-tax payroll deductions.
